Waiver Form

Camper Name_______________________________Nickname___________________ Guest assumes and acknowledges the risks in participating in camp activities. This includes but is not limited to existing and changing weather and trail conditions: rocks, stumps, trees: variations in terrain, collisions with natural or man-made objects. I agree to assume those risks and release and hold Survivor Camp, National Tennis School Inc., Catholic Board and Camp Fortune Ski Hill and their officers, directors, employees, agents and affiliates harmless from, and waive any claim against the Survivor Camp, as to any injury that may occur to the guest while attending Survivor Camp.

Guest hereby grants permission to Survivor Camp and Catholic Board. By signing below, guest indicates that they understand and accept the above releases/conditions, and agree to honor Survivor Camp rules, policies and safety regulations.
